ATHLETIC REGISTRATION

There are two steps required for athletes to register to participate on an athletic team at St. Mary's Academy.
STEP 1: Register Online at FinalForms
Before a student may participate in a sport, the athlete and parent must complete the online athletic registration and permission forms for St. Mary's Academy athletics at the website FinalForms.
 

After submitting a one-time family registration with FinalFormsparents and their athletes must complete the registration for their athlete(s) AND complete all required forms. Payment can be made for the seasonal participation through FinalForms, by cash/check, or through their FACTS account. Families who wish to pay athletic fees by credit/debit card must do so by Monday, August 17th. Please note that a $10 credit card processing fee applies to all credit/debit card payments. Families who wish to pay by cash or check should drop the cash/check off to the athletics office prior to the start of tryouts. Fees not paid by credit/debit card or cash/check by the 17th will automatically be charged through their FACTS account following the conclusion of the tryout period.

STEP 2: Submit a Physical Evaluation Form

Each student-athlete must also download the state mandates, OSAA printed form School Sports Pre-Participation Examination Form (current form revised in April 2023), undergo a physical examination by a physician licensed in the State of Oregon, and upload the form (signed by the physician) into your FinalForms account. This form is valid for 24 months (2 years) from the date signed; a current physical must be on file with the Athletic Office during each year the student instend to participate on any SMA athletic team(s). 

No student will be permitted to participate in athletic tryouts, practices, or games until registration is complete on the FInalForms site AND the annual Pre-Participation Examination Form is uploaded into their FinalForms account. 

All students interested in participating in a 2026 FALL sport must complete the following steps by WEDNESDAY, AUGUST 12TH (prior to tryouts/practice which begin on Monday, August 17th), to be eligible for participation. View the 2026-27 SMA Student-Athlete Handbook for details regarding all athletic and school policies.       

ATTENDANCE 

  • Students involved on SMA athletic teams are required to attend a full day of classes prior to any team activity.
  • Students missing more than 10 minutes of class including zero period, are not allowed to compete/practice/take part in any team functions for the remainder of the day, including: attending any practices, competitions, team meetings or gatherings, team travel, team pictures or sitting on the team bench at a competition.  
  • Additionally, student-athletes must participate in physical education class activities in order to be eligible for athletic participation. Modifications to activity may be made with approval of the instructor to accommodate the student-athlete's limitations.  
  • Medical, dental and counseling appointments, funerals and family emergencies are the only accepted reasons for absence from class that will still allow a student to participate in athletic practices or competitions on the same day. 
  • Medical, dental or counseling appointments must be documented by a note from the provider or provider’s office and a photograph of the note emailed directly to the SMA Athletic Department at athletic.medical.docs@smapdx.org.  The note should include clearly the date and time of the appointment as well as the provider's name. All other absences seeking exception to this policy must be cleared a minimum of 24 hours in advance with the VP of Student Life.
