All forms must be submitted to St. Mary's athletic department prior to participating in any athletic activity including: tryouts, practice, or competition. For more information about St. Mary's athlete rules and regulations, guidelines and policies, please refer to our Student Athletes' Handbook.
BEFORE PARTICIPATING IN ANY SPORT, YOU MUST BE ELIGIBLE.
- Check your athletic eligibilty status on PASS (for sophomores, juniors, and seniors) or click here for more details on eligibility.
- Make sure to have the following forms turned in to the Athletic Department Office prior to the first day of tryouts in order to be eligible.
Parent Permission & Athlete's Contract (good for current school year)
Emergency Medical History Card (good for current school year)
School Sports Pre-Participation Examination Form (good for two years from date of last exam)
- St. Mary's is offering Athletic Physicals for $5.00 (2007 Date pending confirmation)

Students participating in athletics at St. Mary’s will be accessed $140 participation fee per season. There are two alternate methods to choose in covering this fee. (Fees will not be assessed or collected until after teams have been formed.)
1.) Pay a $140 fee (this is billed with your tuition statement)
2.) Participate in Entertainment Book ‘08 sales (see details below)
