To request a transcript, please mail or fax a signed request that includes the following information:

1. Your legal name at the time of graduation.
2. The year you graduated or the last year you attended St. Mary’s Academy.
3. Your date of birth.
4. The number of official (sealed) and/or unofficial transcripts needed.
5. The address or addresses to which the transcripts should be mailed or the date you will pick them up at the school.

Email requests are acceptable only with a signature affixed just as for a written request.  Email requests may be addressed to patty.scruggs@stmaryspdx.org.
