All forms should be submitted together to St. Mary's Academy with the $50 nonrefundable application fee.
These forms are to be given to your current school for completion and submission to St. Mary's Academy. It is important that the applicant's parents sign the forms for release of information and indicates that they should be sent to St. Mary's Academy.
